1. Use the RGB Mode with 8 Bits color. These settings can be checked by going to Image->Mode.
2. Use 72dpi resolution or higher. The action won't work if you use a photo less than 72dpi. This can be fixed by going to Image->Image Size, and increasing the resolution value.
3. Adjust the Opacity to 100%. The Opacity of your brush should be set to 100%. Move the slider at 100%.
